Calvary Youth Registration

Register someone for Calvary Youth. Start by registering YOUR name - the adult(s) in the household, then you will be prompted to add the information for your child(ren). We’ll text you if your child is needed during the program, so leave a cell phone where you can be reached. 

Registering students:

When you click to register a student, a form will open which will include a section on Medical Note. Use this to indicate any food allergies. If none, leave blank.

The Fine Print

Below is the consent and information section (aka "the fine print"). By checking each box you are acknowledging that photos are taken in and around the church building, that we receive your child's information for the purpose of maintaining their yearly records, and that in the event of an emergency, Calvary may be required to call 911 for your child, or to offer your child a bandaid or ice pack.

From time to time we will take photos in the ministries and programs of Calvary Burlington. Photos are used in brochures or promotional material, on church-wide ministry media, newsletters, website, and social media.

If you do not want your child's image used, you may contact Mike Sanders at mike@calvaryburlington.ca. Note we are a public space and we cannot guarantee that your child will not appear in the background of photos, or in photos that others choose to take and post.

Calvary Burlington is collecting and retaining this personal information for the purpose of enrolling your Child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your Child, and to inform you of program updates and upcoming opportunities at our Calvary Burlington. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish to view your Child’s information, please contact us.

The safety of your Child is our primary concern. Precautions will be taken for their well-being and protection. I, one of the parents or guardians named above, authorize the Program Leader or Calvary Burlington Program Personnel to sign a consent for medical treatment and to authorize any physician or hospital to provide emergency medical assessment, treatment or transportation for the participant named above (bandaids and ice packs; calling 911; authorizing travel by ambulance if deemed necessary by EMS).

Parents are always contacted immediately if a child is injured to the extent that a bandaid or cold compress is required, even if the child is well enough to remain in program.

According to our Best Practices Policy, 911 is always called in cases of: Danger to self or others, Unconsciousness or an altered level of consciousness, Difficulty breathing or no signs of breathing, Persistent chest pain or pressure, Significant bleeding (500ml), Seizures, Severe headaches or slurred speech, Injuries to the head and/or spine, Blood in the vomit, urine or stool, Imminent childbirth.

I/we undertake and agree to indemnify and hold harmless Program Personnel, Calvary Burlington, and its Leaders from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Calvary Burlington, as well as of any medical treatment authorized by the supervising individuals representing the Calvary Burlington. This consent and authorization is effective only when participating in or traveling to events sponsored by Calvary Burlington.
